The following problem areas were observed which require remedial action within 1 year of notification to the owners 1 Repair the deteriorated portions of the spillway, spillway walls concrete core wall and capping 2 Monitor the seepage observed at the toe of the earth embankment at bi-weekly intervals with the aid of weirs 3 Remove the trees and brush on the slopes and abutments of the dam. Provide a program of periodic cutting and mowing and 4 Provide a program of periodic inspection and maintenance of the dam and appurtenances, including yearly operation and lubrication of the reservoir drain system, and monitoring of the sedimentation behind the spillway.
