Objective To report a case of H. parainfluenza endocarditis presenting as diplopia, hypertropia, visual field loss, and skew deviation. Background Infective endocarditis IE carries a high incidence of morbidity and mortality and may present diversely. Early neurologic presentations are encountered rarely. According to a prospective study of 2781 patients, vascular embolic events were encountered in only 17 and Roth spots in 2 of patients.
