Blastomyces dermatitidis is a dimorphic fungus endemic to the Mississippi River valley. Severe pulmonary disease secondary to blastomycosis is rare in immunocompetent individuals. We describe a case of chronic pulmonary blastomycosis complicated by large pulmonary cavitation which was originally misdiagnosed as active pulmonary tuberculosis.
