Abstract:
The program encompassed discontinuity stress analysis of the large, welded vessel, an evaluation of ultrasonic inspection methods by post-test sectioning, and a detailed failure analysis. At 800 psi, 85 percent of the planned cyclic pressure level, the nozzle adapter-to-case bolts failed secondary origins were found in the pin joint and the nozzle adapter forging. Concurrent examinations of the previously used bolts by LPC, Mellon Institute, and SPS Laboratories agreed in findings of evidence of stress corrosion in the 260,000-psi strength level H-11 bolts. The discontinuity stress analysis showed a forty percent indicated stress riser around the longitudinal welds caused by sea-gulling contour deviations.
