Abstract : The nursing case management model of patient care delivery is examined throughout this paper. A historical perspective and key forces that led to its emergence on the healthcare scene are included. Two distinct models of nursing case management, including components and distinguishing features of each program are discussed as well as recommendations for their implementation. A study of both models of nursing case management shows unique strengths in each and the potential for broad applications. The relevance of nursing theory to the nursing case management concept is also explored
